This is the first in a series of Summa blog posts delivering an overview of Salesforce.com?s foray into the Social Enterprise.     (AP) ? The survival of President Barack Obama's health care overhaul rests with a Supreme Court seemingly split over ideology and, more particularly, in the hands of two Republican-appointed justices.

    Chief Justice John Roberts and Justice Anthony Kennedy put tough questions to administration lawyers defending the health care law during three days of arguments that suggested they have strong reservations about the individual insurance requirement at the heart of the overhaul and, indeed, whether the rest of the massive law can survive if that linchpin fails.

    But Roberts and Kennedy also asked enough pointed questions of the law's challengers to give the overhaul's supporters some hope. In any event, justices' questions at arguments do not always foretell their positions.

    The court's decision, due in June, will affect the way virtually every American receives and pays for health care and surely will reverberate in this year's campaigns for president and Congress. The political effects could be even larger if the court votes 5-4 with all its Republican-appointed justices prevailing over all the Democratic appointees to strike down the entire law, or several important parts of it.

    Not since 2000, when the court resolved the Bush v. Gore dispute over Florida election returns that sealed George W. Bush's election as president, has a Supreme Court case drawn so much attention.

    The court wrapped up public arguments Wednesday on the overhaul, which aims to extend health insurance to most of the 50 million Americans now without it. The first and biggest issue the justices must decide is whether the centerpiece of the law, the requirement that nearly all Americans carry insurance or pay a penalty, is constitutional.

    Wednesday's morning session was unusual in that it assumed, for purposes of argument, a negative answer to that central question. What should happen to other provisions, the justices and lawyers debated, if the court strikes down the requirement? If the justices are following their normal practice, they had not even met to take a preliminary vote in the case before all argument concluded.

    Questions at the court this week showed a strong ideological division between the liberal justices who seem inclined to uphold the law in its entirety and the conservative justices whose skepticism about Congress' power to force people to buy insurance suggests deep trouble for the insurance requirement, and possibly the entire law.

    The divide on the court reflects a similar split in public opinion about the law, which Congress approved two years ago when Democrats controlled both the House and Senate.

    Liberal and conservative justices alike appeared to accept the administration's argument that at least two important insurance changes are so closely tied to the must-have-coverage requirement that they could not survive without it: provisions requiring insurers to extend coverage to people with existing medical problems and limiting how much those companies can charge in premiums based on a person's age or health.

    Less clear was whether the court would conclude that the entire law, with its hundreds of unrelated provisions, would have to be cast aside.

    The justices also spent part of the day considering a challenge by 26 states to expansion of the federal-state Medicaid program for low-income Americans ? an important feature which alone was expected to extend coverage to 15 million people and which no lower court has rejected. The conservative justices appeared open to the states' argument that the expansion is unconstitutionally coercive.

    Solicitor General Donald Verrilli Jr. took a few seconds at the end of the Medicaid argument to make a final plea for the court to uphold the entire law, which he said would "secure the blessings of liberty" for millions of Americans by providing them with affordable health care.

    Verrilli told the court that Congress had made a policy decision to fight the high cost of medical care through the new law. "I would urge the court to respect that judgment," he said.

    Paul Clement, the lawyer for the states challenging the law, answered that it would be a strange definition of liberty to make people who may not want it buy health care insurance. And he called Congress' threat to cut all Medicaid funding from states that refuse to expand the program "a direct threat to our federalism."

    (AP) ? With Republicans locked in a contentious and expensive primary, President Barack Obama has spent a small fortune in recent months to build and maintain a campaign operation that is larger, more diverse and more focused on November's general election than any of his opponents' organizations.

    Republican contenders like Mitt Romney and Rick Santorum have surely been watching their expenses during their primary elections: millions here for ad spending, millions there for travel, rallies and consulting fees. What's left keeps the lights on, the phones ringing and the staff paid.

    But Obama, who faces no serious challenger for the Democratic nomination, has sunk his cash into an expansive brick-and-mortar operation with offices in nearly every state. His campaign has spent more than $135 million on operations through February, according to an Associated Press analysis of Federal Election Commission records. That's about $3 million more than all his GOP challengers combined.

    Republicans bristle over reports that Obama's paid staff exceeds 500, many of whom work in the campaign's Chicago headquarters.

    "I think the campaign is single-handedly trying to lower the unemployment rate by hiring field staff," Romney political director Rich Beeson said. "When they point to the fact about how many people they've got hired and how many offices they've got, they're just trying to distract people from the reality of (how) they're going to have a heck of a time finding people to get out and vote for him."

    A review of Obama's balance sheets reveals a small army of paid staffers trying to help the Democratic president win a second term. Campaign filings list more than 330 paid staffers in Chicago and 200 more spread across the country ? payroll costs that exceed $6.3 million during the last two months alone.

    The numbers also suggest signs of stress.

    Obama's team has spent more than $7 million since January on fundraising-related expenses like postage, printing and telemarketing, in some cases to contact the same kind of low-dollar donors who supported Obama four years ago. The campaign also spent millions more on expenses like online advertising and consulting, which in some cases can be tied to fundraising.

    With offices in nearly every state, the campaign also faces rising overhead. Through the first two months of the year, Obama spent approximately $1.1 million on computer equipment, $435,000 in rent and utilities, $305,000 on telephones, and $19,000 on office supplies, federal filings show.

    "We're building the largest grass-roots campaign in history," campaign spokesman Ben LaBolt said. "You can see it here, but it's really happening in the states."

    The core of Obama's operation is packed into the sixth floor of Chicago's Prudential building, where 300 staffers sit side by side at long rows of tables, working from laptops and cellphones. Colorful college pennants hang from the ceiling and often represent key swing states: the University of North Carolina, Ohio State and the University of Michigan. Need a designer T-shirt or bumper sticker? A room managed by two staffers houses a swelling collection of campaign memorabilia for sale.

    In one corner, more than a dozen workers field questions from journalists scattered across the country. Elsewhere, others coordinate media appearances for Obama's high-profile supporters. Other staffers focus on fundraising, voter identification, social media and campaign-finance reporting.

    Beyond the Windy City, Obama's campaign said it's already opened five field offices in Arizona, a state it expects to be increasingly competitive in the fall. The campaign is also taking advantage of party resources there, relying in part on the state's Democratic Party for staffing, phones and computer equipment, records show.

    Obama's operation had $84.7 million in cash-on-hand by Feb. 29. But the Romney campaign ? which is hardly hurting for cash after raising about $74.8 million ? says it's not impressed.

    Romney's Boston-based campaign is a fraction of the size of Obama's, although its organization dwarfs its Republican competitors. With around 100 paid staffers, Romney has spent more than $180,000 in rent and utilities since early January in at least 16 states, records show, from Massachusetts to Utah. Santorum, whose national headquarters was technically a post office box until this month, spent just $19,000 since the beginning of the year on rent-related expenses.

    Romney's ability to compete against Obama's growing organization has become a selling point on the trail.

    "As Republican primary voters in Illinois, we have an opportunity to look at the field of candidates, and look at them and say, 'Who can go toe-to-toe not just with Barack Obama, but the Chicago machine that is his operation?'" Rep. Aaron Schock, a top Romney supporter in Illinois, said recently. "We cannot afford to nominate someone on our ticket who cannot withstand the barrage, who does not have the organizational strength and fortitude to go toe-to-toe with Barack Obama."

    Obama's massive campaign is part of what drives supporters to give money to "super" political action committees popping up this election cycle. Major super PACs like American Crossroads, which supports Republicans, and Restore Our Future, which supports Romney, have tens of millions of dollars apiece to help, thanks to federal court rulings that have stripped campaign-finance rules to allow unlimited ? and, at times, effectively anonymous ? donations from billionaires, corporations and labor unions.

    "As a general rule, the big institutional money goes inordinately to incumbents regardless of party, while outside money plays the role of balancing out the party in power or stopping a specific initiative," Crossroads spokesman Jonathan Collegio said. GOP-leaning super PACs have spent more than $50 million on TV ads this election; Crossroads is largely waiting to spend its money until the general election.

    Whatever the case, the Democrats' current financial advantage is something Obama's campaign isn't taking for granted. Obama changed course last month in his criticism of super PACs and began encouraging big-money supporters to give to Priorities USA Action, a group working in his favor.

    The Republican Party has opened offices in three states this month and plans to expand to four others in April.

    It hasn't made its official open-source debut yet, but Android 4.0.4 should be hitting the repositories very soon. Leaks of the next Ice Cream sandwich update have been circulating since early February, and Moto's Xoom was given the first taste today. Now Jean-Baptiste Queru, an engineer on the mobile OS, has posted over at the Android Open Source Project forums to let us know that the appropriate files are being pushed as we speak. Of course, this doesn't mean that 4.0.4 will hit your handset any time soon... or ever, for that matter.     Two faulty dances lead to elimination on 'DWTS'

    The judges couldn't find much to praise about Martina's jive on Monday.

    Those were ?Dancing With the Stars? host Tom Bergeron?s words to pro Tony Dovolani as the hoofer and his celeb partner, tennis legend Martina Navratilova, became the first pair eliminated from season 14 of the ballroom bash.

    Sorry indeed. For once again, the likable pro was paired with a star who couldn?t take him farther in the competition. (But hey, at least Martina was very pleasant, unlike past partners who left the pro angry and frustrated.)

    For her premiere dance, Martina delivered a flawed, 20-point foxtrot head judge Len Goodman called ?too careful.? Her week two performance, the jive, lacked attack and wasn?t much better. In fact, the judges gave her a paltry score of 17. Judge Bruno Tonioli even told her, ?I think that serve went into the net. You couldn?t keep up with the dance.? And Carrie Ann Inaba couldn?t find much to praise about it. ?That was sad,? the judge said before adding, ?I loved your attitude!? Ouch.

    And the tennis great knew she had screwed up. ?I blew it like I haven?t blown it!? she said after her performance. ?I can?t believe I did that.?

    Not surprisingly, her subpar performances landed her at the bottom of the leaderboard both weeks, with a total score of 37 out of 60.

    Though her dance moves lacked the grace and precision that she delivered on the tennis courts, Martina had poise to spare and didn?t play the blame game when she said her goodbyes. ?It was an amazing experience,? she said of her short stint on ?Dancing.? ?I just wish it had lasted longer. I gave it everything I had, but I blew it yesterday.?

    On the subject of ?blowing it,? fans of the show might also note that the judges screwed up the first two weeks of this season by appearing to grade certain pairs by different standards. (Just check out the comments on our Facebook page!) Yes, we?re talking about Melissa Gilbert and pro partner Maks Chmerkovskiy, who were criticized for their ambitious and not-all-that-bad dances, while others who made more obvious goofs (including missed steps) scored higher and received encouragement from the panel.

    But it was the actress and the ballroom bad boy who were triumphant for this first elimination round. The two were the first pair Tom announced as safe for the week. With that revelation, Maks broke into a huge grin and excitedly pumped his fist into the air. It was perhaps the happiest we?ve seen the pro since the judges started nitpicking his partner in week one.

    He wasn?t the only one to be uber excited to advance to the third week. When ?View? co-host Sherri Shepherd learned her fate, she busted out with a wild celebratory dance that left the others in awe of her energy.

    And speaking of energy, expect the ballroom to sizzle next week! Cuban actor William Levy, who earned many new fans when he debuted on ?Dancing? with one of the sexiest performances ever seen on the show, revealed he?ll be dancing the salsa for personal stories week. Muy caliente!

    The H1N1 flu pandemic in 2009 underscored weaknesses in methods widely used to diagnose the flu, from frequent false negatives to long wait times for results. Now Boston University researchers have developed a prototype of a rapid, low-cost, accurate, point-of-care device that promises to provide clinicians with an effective tool to quickly diagnose both seasonal and pandemic strains of influenza, and thus limit the spread of infection.

    Boston University Biomedical Engineering Associate Professor Catherine Klapperich led the team of engineering and medical researchers that developed the disposable microfluidic chip the size of a microscope slide that aims to take the place of expensive, time-consuming, laboratory-based diagnostic tests now in use. The chip essentially miniaturizes the RT-PCR test, now considered the gold standard in flu detection, producing faster, cheaper and accurate results.

    The researchers collected clinical nasal specimens and placed them on the chip, which extracts RNA from the Influenza A virus, converts it into DNA and replicates the sample in sufficient quantities so it can be detected by a table-top external reader. The chip produced results that matched the accuracy of the lab-based tests.

    "We wanted to show that our technique was feasible on real-world samples prepared on the chip," said Klapperich. "Making each chip single-use decreases the possibility of cross-contamination between specimens, and the chip's small size makes it a good candidate for true point-of-care testing."

    The microfluidic chip also proved far more effective than other commonly used flu diagnostic tests including viral culture, a lab procedure requiring up to a week to produce results; rapid immunoassays, which work like pregnancy tests but were only 40 percent reliable in detecting the presence of a flu virus in this study; and direct fluorescent antigen testing (DFA), a more accurate but labor-intensive process in which medical personnel prepare and interpret samples stained with fluorescent antibodies.

    "The new test represents a major improvement over viral culture in terms of turn-around time, over rapid immunoassay tests in terms of sensitivity (ability to detect the virus from minimal sample material) and over DFA and RT-PCR in terms of ease of use and portability," Klapperich observed.

    Ultimately seeking to enable clinicians to use their microfluidic chips for frontline flu virus detection, the researchers next plan to optimize their method so that it can produce results in a third less time (an hour) with chips that cost half as much to make (five dollars). In addition, they are exploring ways to develop a lower cost external reader that's no bigger than a clinical digital thermometer.

    New way to abate heart attacks before patients get to the hospital

    ScienceDaily (Mar. 27, 2012) ? Paramedics can reduce someone's chances of having a cardiac arrest or dying by 50 percent by immediately administering a mixture of glucose, insulin and potassium ("GIK") to people having a heart attack, according to research presented March 27 at the American College of Cardiology's 61st Annual Scientific Session.

    The study showed that patients who received GIK immediately after being diagnosed with acute coronary syndrome -- which indicates a heart attack is either in progress or on the way -- were 50 percent less likely to have cardiac arrest (a condition in which the heart suddenly stops beating) or die than those who received a placebo, although the treatment did not prevent the heart attack from occurring. Over the first month following the event, patients who received GIK were 40 percent less likely to have cardiac arrest, die or be hospitalized for heart failure.

    The effect was even more striking for patients with ST-elevation heart attacks, which require immediate treatment. For those patients, immediate GIK was associated with a 60 percent reduction in cardiac arrest or death.

    "When started immediately in the home or on the way to the hospital -- even before the diagnosis is completely established -- GIK appears to reduce the size of heart attacks and to reduce by half the risk of having a cardiac arrest or dying," said Harry P. Selker, MD, MSPH, executive director of the Institute for Clinical Research and Health Policy Studies at Tufts Medical Center, who led the study with Joni Beshansky, RN, MPH, co-principal investigator and project director. "Acute coronary syndromes represent the largest cause of death in this country. GIK is a very inexpensive treatment that appears to have promise in reducing those deaths and morbidity."

    The cost of the treatment is about $50.

    "Because the trial is the first to show GIK is effective when used by paramedics in real-world community settings, it could have important implications for the treatment of heart attacks," Dr. Selker said. Previous clinical trials have shown no consistent effect, likely because the GIK was given too late to help. This study, the "IMMEDIATE Trial," was the first to test the effectiveness of administering GIK at the very first signs of a threatening heart attack, in the community, rather than waiting hours until the diagnosis was well-established at a hospital, as done in previous clinical trials.

    "We wanted to do something that is effective and can be used anywhere," said Dr. Selker. "We've done a lot of studies of acute cardiac care in emergency departments and hospitals, but more people die of heart attacks outside the hospital than inside the hospital. Hundreds of thousands of people per year are dying out in the community; we wanted to direct our attention to those patients."

    The researchers trained paramedics in 36 Emergency Medical Services systems in 13 cities across the country to administer GIK after determining that a patient was likely having a threatened or already established heart attack using electrocardiograph-based ACI-TIPI (acute cardiac ischemia time-insensitive predictive instrument) and thrombolytic predictive instrument decision support that prints patient-specific predictions on the top of an electrocardiogram. The paramedics used these predictions to decide if a patient would likely benefit from treatment. There were 911 patients randomized to receive either the GIK treatment or a placebo.

    Administering GIK immediately also reduced the severity of the damage to the heart tissue from the heart attack. On average, 2 percent of the heart tissue was destroyed by the heart attack in people receiving GIK, compared with 10 percent in those who received the placebo. Although a significant proportion of suspected heart attacks are later determined to be false alarms (23 percent in this study), administering GIK does not appear to cause any harmful effects in such patients.

    The research team will follow up with study participants at six and 12 months to evaluate the longer-term benefit of the GIK treatment.
